Provide the true and required information in all the form fields as requested by the e-form. Carefully review your entries and ensure they are all correct before you submit.
Participants are to come to the training venue with their Nursing Practice License, payment tellar and a valid means of identification (National ID, Driver's Licnse, International Passport or Voter’s Card) for verification. Note that breastfeeding Mothers to come with Nannies.
